Контрольована гіпотензія при функціональній ендоскопічній хірургії приносових пазух у дітей

Стаття містить огляд літературних наукових джерел, згаданих у бібліографічній базі даних Medline, на платформі PubMed, Web of Science, Scopus, щодо використання контрольованої гіпотензії під час проведення функціональної ендоскопічної хірургії приносових пазух у дітей. Проаналізовано анатомічні та функціональні особливості педіатричних пацієнтів, що впливають на вибір цільових параметрів та безпеку проведення контрольованої гіпотензії. Проведено порівняльну характеристику препаратів різних груп, що ґрунтуються на сучасних дослідженнях ефективності контрольованої гіпотензії при функціональній ендоскопічній хірургії приносових пазух. Акцентовано увагу на оцінці ефективності препаратів для контрольованої гіпотензії, що базується не тільки на здатності знижувати артеріальний тиск, а й на створенні оптимального хірургічного поля, мінімізації крововтрати, потенціюванні анестезії, забезпеченні задовільної перфузії органів та впливі на тривалість анестезії та операції в цілому.
Article presents a review of scientific literature indexed in the MEDLINE database via the PubMed platform, as well as in the Web of Science and Scopus databases, regarding the use of controlled hypotension during functional endoscopic sinus surgery in pediatric patients. Particular attention is paid to the anatomical and functional characteristics of children that may influence the selection of target hemodynamic parameters and the safety profile of controlled hypotension. A comparative analysis of pharmacological agents from different classes was conducted based on contemporary studies evaluating efficacy of controlled hypotension in functional endoscopic sinus surgery. Emphasis is placed on assessing the effectiveness of drugs for controlled hypotension, which is based not only on the ability to lower blood pressure, but also on creating an optimal surgical field, minimizing intraoperative blood loss, potentiating anesthetic effects, maintaining adequate end-organ perfusion, and influencing the overall duration of anesthesia and surgery.
