Актуальність. Автоімунний гастрит (АІГ) — це хронічне імуноопосередковане захворювання, яке характеризується руйнуванням кислотопродукуючої слизової оболонки (СО) шлунка, що обумовлено утворенням автоантитіл до парієтальних клітин та/або внутрішнього фактора, це призводить до атрофії кислотопродукуючої СО, гіпо-/ахлоргідрії в шлунку, порушення всмоктування вітаміну В12, заліза, розвитку перніціозної анемії, підвищеного ризику розвитку нейроендокринних пухлин. Діагностика АІГ може бути несвоєчасною і утрудненою, оскільки шлункові симптоми виражені слабо, а ендоскопічна і гістологічна картина на початкових стадіях неспецифічна. Мета роботи: провести аналіз сучасної наукової літератури стосовно АІГ і визначити проблеми діагностики та ведення хворих на АІГ з різними варіантами перебігу на прикладі клінічних випадків. Матеріали та методи. Наведено два власних клінічних спостереження. Клінічний випадок 1: жінка 60 років, з наявністю диспептичних симптомів, тривалий час спостерігалась з діагнозом «атрофічний гастрит», і лише через 8 років був встановлений діагноз АІГ. Клінічний випадок 2: жінка 42 років, в анамнезі була виражена анемія, атрофічний гастрит та рецидивні нейроендокринні (карциноїдні) пухлини шлунка; лише через 5 років був встановлений діагноз АІГ. Проведено детальний аналіз і обговорення клінічних випадків. Висновки. Автоімунний гастрит є захворюванням, яке погіршує якість життя пацієнтів й часто діагностується несвоєчасно, про що свідчать наведені клінічні випадки. Наявність у пацієнта анемії, диспептичних скарг, супутніх автоімунних захворювань (в першу чергу щитоподібної залози), неврологічних порушень потребує виключення АІГ. Своєчасне встановлення діагнозу АІГ є важливим для запобігання та ефективного лікування його клінічних проявів і можливих несприятливих наслідків. Менеджмент пацієнтів з АІГ полягає в більш ретельному ендоскопічному й морфологічному спостереженні з метою своєчасного виявлення таких ускладнень, як нейроендокринні пухлини, передракові зміни й рак шлунка.

Background. Autoimmune gastritis (AIG) is a chronic immune-mediated disease characterized by the destruction of the acid-producing gastric mucosa, which is caused by the formation of autoantibodies to parietal cells and/or intrinsic factor, leading to atrophy of the acid-producing CO, hypo-/achlorhydria in the stomach, impaired absorption of vitamin B12, iron, the development of pernicious anemia, and an increased risk of neuroendocrine tumors. Diagnosis of AIG may be delayed and difficult, as gastric symptoms are mild and endoscopic and histological findings are nonspecific in the early stages. The purpose of the work is to analyze the modern scientific literature on AIG and to determine the problems of diagnosis and management of patients with AIG with different course variants using clinical cases as an example. Materials and methods. Two of our own clinical observations are presented. Clinical case 1: a 60-year-old woman with dyspeptic symptoms was observed for a long time with a diagnosis of atrophic gastritis, and only after 8 years, a diagnosis of AIG was established. Clinical case 2 was a 42-year-old woman with a history of severe anemia, atrophic gastritis, and recurrent neuroendocrine (carcinoid) tumors of the stomach; only after 5 years, a diagnosis of AIH was made. A detailed analysis and discussion of clinical cases were conducted. Conclusions. Autoimmune gastritis is a disease that worsens the quality of life of patients and is often diagnosed late, as evidenced by the two clinical cases. The presence of anemia, dyspeptic complaints, concomitant autoimmune diseases (primarily of thyroid gland), neurological disorders requires the exclusion of AIG in a patient. Timely diagnosis of AIG is important for the prevention and effective treatment of its clinical manifestations and possible adverse consequences. Management of patients with AIG consists of more careful endoscopic and morphological observation for the timely detection of complications such as neuroendocrine tumors, precancerous changes, and gastric cancer.
