Вплив кальцитоніну та інфрачервоного лазерного опромінення на сироваткові маркери ремоделювання кісткової тканини й запалення у щурів за умов експериментального дефекту кістки

Актуальність. Оптимізація репаративного остеогенезу після травматичного ушкодження кісток залишається важливим завданням регенеративної медицини й експериментальної ортопедії, особливо в умовах остеопорозу і порушень зрощення. Сироваткові маркери формування, резорбції та запалення кісткової тканини широко використовують для оцінки перебігу репаративних процесів й ефективності терапевтичних втручань. Тому пошук нових підходів до модулювання репаративних процесів у кістковій тканині становить значний науковий і клінічний інтерес. Серед таких методів застосування кальцитоніну й інфрачервоного лазерного опромінення має потенційний остеопротекторний та імуномодулюючий вплив. Мета: оцінити зміни сироваткових маркерів ремоделювання кісткової тканини та запалення після експериментального ушкодження кісток щурів, а також визначити вплив кальцитоніну й інфрачервоного лазерного опромінення на ці показники. Матеріали та методи. Репаративний остеогенез досліджували в експерименті in vivo на 30 щурах лінії Wistar, яким під загальною анестезією створювали травматичні дефекти в середній третині діафіза великогомілкової кістки. Випадковим чином їх розподілили на п’ять груп (по 6 у кожній): 1-ша — інтактні тварини; 2-га — неліковані щури з травматичним ушкодженням кістки (контроль); 3-тя — тварини, яким одразу після травми проводили локальне опромінення інфрачервоним лазером; 4-та — щури, які отримували кальцитонін; 5-та — тварини, у яких застосовували комбінацію кальцитоніну й інфрачервоного лазерного опромінення. Визначено сироваткові концентрації кальцію та неорганічного фосфату, активність лужної фосфатази і рівень дезоксипіридиноліну (DPD) як маркерів ремоделювання кісткової тканини. Рівні інтерлейкіну (IL) 1β та IL-10 у сироватці крові оцінювали як показники системної запальної відповіді, пов’язаної з експериментальним кістковим ушкодженням. Дослідження проводили на 7-му та 28-му добу після операції. Результати. Терапевтичні втручання сприяли відновленню сироваткової концентрації неорганічного фосфату до рівнів, близьких до контрольних, причому найбільш виражений ефект спостерігали при комбінованому застосуванні кальцитоніну та лазерного опромінення. Подібні зміни виявлено щодо концентрації кальцію в сироватці крові, яка наближалася до показників інтактних тварин, особливо в групі комбінованої терапії. Інфрачервоне лазерне опромінення як самостійно, так і в комбінації з кальцитоніном вірогідно підвищувало активність лужної фосфатази на 7-му добу та знижувало — на 28-му добу порівняно з нелікованими тваринами, що свідчить про посилення активності остеобластів та прискорене формування кісткового матриксу на ранніх етапах репаративного остеогенезу. Крім того, застосування кальцитоніну й лазерного опромінення вірогідно знижувало концентрацію DPD у сироватці крові на 28-му добу, тоді як їх комбіноване застосування нормалізувало рівень DPD до показників інтактних тварин, що демонструє пригнічення процесів кісткової резорбції. На 7-му добу в групах лікування виявлено виражене зростання концентрації IL-10, яка перевищувала параметри нелікованих тварин у 2,4 раза, що може відображати активацію протизапальних механізмів, залучених до репарації тканин. На 28-му добу рівень IL-10 знижувався та наближався до контрольних значень, що свідчило про завершення запальної відповіді. Загалом комбіноване застосування кальцитоніну й інфрачервоного лазерного опромінення продемонструвало найбільш сприятливий вплив як на показники ремоделювання кісткової тканини, так і на маркери запалення. Висновки. Застосування кальцитоніну, інфрачервоного лазерного опромінення та, особливо, їх комбінації чинить сприятливий вплив на репаративний остеогенез і модулює системну запальну відповідь після експериментального ушкодження кісток у щурів. Комбінована терапія характеризувалася найбільш вираженою нормалізацією показників мінерального обміну, пригніченням маркерів остеорезорбції та відновленням цитокінового балансу, що свідчить про її потенціал як перспективної стратегії посилення репарації кісткової тканини.
Background. Optimization of reparative osteogenesis following traumatic bone injury remains an important challenge in regenerative medicine and experimental orthopedics, particularly in the context of osteoporosis and impaired bone healing. Serum markers of bone formation, bone resorption, and inflammation are widely used to evaluate the course of bone repair and the efficacy of therapeutic interventions. Therefore, the search for novel approaches capable of modulating reparative processes in bone tissue is of considerable scientific and clinical interest. Among such approaches, calcitonin administration and infrared laser irradiation have shown potential osteoprotective and immunomodulatory properties. The purpose of the study was to evaluate changes in serum markers of bone remodeling and inflammation following experimental bone injury in rats and to assess the effects of calcitonin and infrared laser irradiation on these parameters. Materials and methods. Reparative osteogenesis was investigated in an in vivo experiment involving 30 Wistar rats with standardized traumatic defects created in the mid-diaphyseal region of the tibia under general anesthesia. The animals were randomly assigned to five groups (n = 6 per group): group 1 — intact animals; group 2 — untreated rats with traumatic bone injury (control); group 3 — animals subjected to local infrared laser irradiation immediately after injury; group 4 — rats treated with calcitonin; and group 5 — animals receiving combined treatment with calcitonin and infrared laser irradiation. Serum concentrations of calcium and inorganic phosphate, alkaline phosphatase activity, and deoxypyridinoline (DPD) levels were determined as markers of bone tissue remodeling. Serum levels of interleukin-1β (IL-1β) and IL-10 were assessed as indicators of the systemic inflammatory response associated with experimental bone injury. Measurements were performed on postoperative days 7 and 28. Results. Therapeutic interventions promoted the restoration of serum inorganic phosphate concentrations toward control values, with the most pronounced effect observed following combined treatment with calcitonin and laser irradiation. Similar changes were detected for serum calcium concentrations, which approached the levels observed in intact animals, particularly in the combination therapy group. Infrared laser irradiation alone and in combination with calcitonin significantly increased alkaline phosphatase activity on postoperative day 7 and decreased its activity by day 28 compared to untreated animals, indicating enhanced osteoblastic activity and accelerated bone matrix formation during the early stages of reparative osteogenesis. Furthermore, calcitonin administration and laser irradiation significantly reduced serum DPD concentrations by postoperative day 28, whereas their combined application normalized DPD levels to those observed in intact animals, suggesting attenuation of bone resorption processes. A marked increase in IL-10 concentration was detected on postoperative day 7 in the treatment groups, reaching values 2.4-fold higher than those in untreated animals, which may reflect activation of anti-inflammatory mechanisms involved in tissue repair. By postoperative day 28, IL-10 concentrations had declined and approached control values, indicating resolution of the inflammatory response. Overall, the combined use of calcitonin and infrared laser irradiation demonstrated the most favorable effects on both bone remodeling and inflammatory markers. Conclusions. Calcitonin administration, infrared laser irradiation, and particularly their combined application exert beneficial effects on reparative osteogenesis and modulate the systemic inflammatory response following experimental bone injury in rats. Combined therapy was associated with the most pronounced normalization of mineral metabolism parameters, suppression of osteoresorption markers, and restoration of cytokine balance, suggesting its potential as a promising strategy for enhancing bone repair.
