Background. This study compared perioperative mean arterial pressure (MAP), hemostatic parameters, and lactate measures between transfusion strategies with and without warm fresh whole blood (WFWB). Materials and methods. This retrospective cohort included 92 male casualties with penetrating combat abdominal trauma and hemorrhagic shock at four Role 2 facilities in 2022–2024: 48 received ≥ 2 WFWB units plus blood components and 44 received transfusion therapy without WFWB. MAP was assessed at eight perioperative time points; laboratory parameters were measured preoperatively and before evacuation. Analyses included between-group comparisons and multivariable linear regression. Results. Injury Severity Score was higher in the WFWB group (30.0 ± 6.5 vs 25.0 ± 4.7; p < 0.001). No statistically significant between-group MAP differences were detected at any time point, including after adjustment for injury severity. Before evacuation, the WFWB group had higher platelet counts (139.4 ± 42.0 vs 108.9 ± 49.7 × 109/L; p = 0.002), lower international normalized ratios (1.38 ± 0.28 vs 1.72 ± 0.32; p < 0.001), and lower lactate concentrations (2.75 ± 0.45 vs 3.45 ± 0.92 mmol/L; p < 0.001). Lactate clearance was 34.0 ± 11.0 % versus 12.1 ± 20.4 %; the adjusted difference was 21.6 percentage points (95% confidence interval 13.9–29.4; p < 0.001). Norepinephrine use was 25.0 versus 31.8 % (p = 0.50); among its recipients, cumulative dose was lower in the WFWB group (48.0 ± 22.0 vs 78.0 ± 28.0 μg/kg; p = 0.005). Between-group differences in laboratory indicators remained statistically significant after adjustment for baseline values and clinical covariates. Total blood product volumes did not differ statistically significantly. Conclusions. WFWB use was associated with more favorable hemostatic and lactate parameters without detected MAP differences. The retrospective design limits causal interpretation.
