Показання до видалення металофіксаторів після консолідації переломів кісток (огляд літератури)

Резюме

Повне зрощення перелому й відновлення функції кінцівки не завжди означає закінчення лікування конкретної травми. Наступним етапом хірургічного втручання, як правило, є видалення внутрішніх конструкцій, за допомогою яких було здійснено остеосинтез. Медичні показання до хірургічного видалення металевих імплантатів визначені недостатньо. Серед хірургів різних країн побутують різноманітні погляди на цю проблему, показання залежать від стану пацієнта, анатомічних ділянок, імплантаційних матеріалів. Проведений аналітичний огляд джерел літератури довів, що біль, функціональні порушення, помітний металофіксатор, можливі проблеми в майбутньому і бажання пацієнтів є відносними показаннями до видалення імплантатів. Абсолютними показаннями до видалення є аваскулярний некроз головки стегнової кістки, глибока інфекція і прорізування імплантату. З огляду на наявність численних ускладнень процедура видалення може бути дуже складною, і її можна рекомендувати лише в симптоматичних пацієнтів після належної усвідомленої згоди.

Complete union of the fracture and restoration of limb function does not always mean the end of the treatment of a particular injury. The next stage of surgical treatment of bone fractures, as a rule, is the removal of internal structures, with the help of which osteosynthesis was carried out. Medical indications for surgical removal of metal implants are poorly defined and there are various points of view on this problem among surgeons of different countries; the indications depend on the patient’s state, anatomical areas, and implant materials. An analytical review of literature sources proved that pain, functional impairment, a visible metal fixator, possible future problems and patient requests are relative indications for the removal of implants. Absolute indications for removal are avascular necrosis of the femoral head, deep infection, and implant perforation. Given the number of complications, the removal procedure can be very difficult and should only be recommended only in symptomatic patients after proper informed consent.
